Moving to Stoughton? Want to try a new pharmacy? Changing insurance? If you answered yes to any one of those questions, it may be time to transfer to McGlynn Pharmacy! Transferring a prescription is easy, just let us know which prescriptions you need transferred whether you come in person, call on the phone or contact us by fax or email. Just let us know and we will finish the rest.
Info we need from you:
-Name and phone number
-Old pharmacy name and number
-Name of prescriptions to transfer
-Rx number from old pharmacy (if available)
If you are new to us you can print off our intake form on your computer and bring it in or fax it to us ahead of time. Everything else we can take care of when you come in to pick up your prescription and of course don't forget your insurance card!
